What documents would I need to include in the application?
  • Proof of Signed and Active Rental Status
  • Documents to evidence loss of wages,  loss of income, reduction in hours, letter of unemployment,  termination letter from employer or self-attestation letter form to document loss of income (if reliable proof of loss of income cannot be secured) as a result of the COVID-19 Pandemic
  • Valid forms of identification for all household members
  • Copies of the Two most recent years of signed income tax (IRS) documents (1040,1098, all schedules) including W-2 forms and 1099 statements for all household members. If a tax document is missing in part or in whole, a transcript or certified copy may be requested from the IRS.
  • A past due water bill
  • A past due rent bill
  • A past due electricity bill
  • A past due internet bill, this assistance does not cover phone or TV.  
  • most recent payroll stubs, tips, wages, or a letter of unemployment (if applicable for any or all household members)
  • Social Security (if applicable for any or all household members)
  • Pension (if applicable for any or all household members)
  • Social Security Disability Insurance (if applicable for any or all household members)
  • Government Assistance (if applicable for any or all household members)
  • Unemployment Benefits (if applicable for any or all household members)
  • Copies of the three most recent months of financial information for all household members (checking, savings, IRS, stocks, bonds, etc)
