Additional Requirements For Certain Businesses

If your profession or occupation is regulated by the Florida Department of Business and Professional Regulations Website (850-487-2252), (engineers, real estate firms, mortgage brokers, restaurants, etc. ) you must attach a copy of your current certification, registration or license with your application.
All Food Service Businesses must obtain approval from the Florida Division of Hotels and Restaurants Website (954-958-5520), Florida Department of Agriculture and Consumer Services Website (800-435-7352) or the State Health Department Website (850-245-4444). You are required to attach a copy of the approved inspection report with your application.
Any business that sells or deals in alcohol or tobacco must obtain a certificate from the Division of Alcoholic Beverages and Tobacco Website at 850-487-1395.
Childcare Facilities are regulated by the Florida Department of Children and Families Website (561-837-5900) and Palm Beach County Health Department Website (561-840-4500).
Food outlets, auto repair, travel agencies, telemarketers, health and dance (ballroom) studios must submit a permit, registration or exemption from the Florida Department of Agriculture and Consumer Services Website (800-435-7352).
Certified Contractors must attach a copy of a Florida Department of Business and Professional Regulation Website (850-487-2252) and/or Palm Beach County Certification Website (561-233-5525).
If your business is based within the incorporated boundaries of Wellington; you are required to possess a Wellington Business Tax Receipt. If your business is not based within the boundaries of Wellington, you must submit a copy of a Business Tax Receipt from the county or municipality where your business is based for registration of your license with Wellington.
Banks, mortgage brokers, finance companies and stockbrokers must be registered with the Florida Office of Financial Regulation Website (850-487-9687). Attach a copy of the state, federal or national license showing the proper business location as stated on this application.
Mobile Food Vendors are regulated by Florida Department of Agriculture and Consumer Services Website (800-435-7352) and must attach your current license to apply.
Cosmetology Establishments & Professionals are licensed by Florida Department of Business and Professional Regulation Website (850-487-2252). A current copy of your license(s) must accompany our application.
Tattoo & Piercing Professionals & Establishments are licensed by the State Health Department Website (850-245-4444). A current copy of your license(s) must accompany your application.
Healthcare professionals are licensed by State Health Department Website (850-245-4444). A current copy of your license(s) must accompany your application.
Veterinary Medicine is licensed by Florida Department of Business and Professional Regulation Website (850-487-2252). A current copy of your license(s) must accompany your application.
Assisted Living Facilities & Home Health Agencies are licensed the Florida Agency for Health Care Administration Website (888-419-3456). A current copy of your license must accompany your application.
